21 Jan 1965 - Accepted into USAF from OnMark at Van Nuys, CA [1]
22 Jan 1965 - Assigned to 1st Air Commando Wing at Eglin AFB.[1]
17 Dec 1965 - 1st Air Commando Wing moved from Eglin AFB to England AFB.[1]
Aug 1966 - Sent to Southeast Asia. Participated in Operation Steel Tiger. Assigned to 606th Special Operations Squadron.
22 Feb 1967 - Lost in action. Shot down by AA while strafing a convoy near Ban Thapachon in southern Laos. Plane made it back to Nakhon Phanom but starboard wing began to burn and crew bailed out. The crew survived and were picked up by helicopter.
